Do you want to help make your community healthier? Here's your chance to help — just by completing a survey.
Every three years, the Morrison-Todd-Wadena Community Health Board, along with CentraCare Health Long Prairie, CHI St. Gabriel's Health, Lakewood Health System, and Tri-County Health Care, work collaboratively to administer a community health survey.
The information gathered by this survey is used to identify health priorities and needs in the region. Past surveys have helped address issues such as substance abuse, food insecurity, access to mental health services, and senior nutrition, according to the board.
In the coming weeks, the Morrison-Todd-Wadena Community Health Survey will be sent to a random sample of household addresses across Morrison, Todd and Wadena counties.
If your household receives the survey, it is designed for one adult age 18 or older to fill out. If you have more than one adult in your household, give the survey to the adult who has most recently had a birthday.
Completing this survey is completely voluntary. All answers to the questions are strictly confidential and no personal information will be linked to any of the responses, according to the health collaborators.
